Choosing Cataract Lens Implants in NY

“Choosing a lens implant for vision correction after cataract surgery is an important decision,” commented NY Cataract Surgeon Satish Modi, M.D. of Seeta Eye Centers in Poughkeepsie, New York. “Not too long ago, the decision on the exact type of lens implant to use was the sole responsibility of your eye surgeon”, explained Andreas Wolter, M.D. “Today, with advanced technology lens implants that can correct astigmatism after cataract surgery or even the  near vision problems caused by the  age related focusing problem presbyopia, we like to discuss the  options with patients so that  together we can choose the most appropriate lens implant for the lifestyle needs of each patient,” remarked Dr. Wolter.

A type of lens implant called an aspheric lens implant seems to give the sharpest, most crisp vision-but only corrects distance vision and requires you to wear reading glasses or bifocals after cataract surgery. This might be fine for a truck driver who needs cataract surgery, but might not be the best choice for a sales clerk who uses his or her eyes at many distances all day. A type of lens implant called a toric lens implant is often selected for cataract patients who have astigmatism correction their eyeglasses. If you have a toric lens implant to correct your vision after cataract Surgery you will still need to correct you near vision to see up close with reading glasses of some sort. Dutchess Senior Aging & Eye Care Talk

The Dutchess County Office of Aging Services will host Andreas Wolter, M.D. to speak on Cataract Surgery, Macular Degeneration (ARMD), Glaucoma and Diabetic eye problems on Thursday September 13, 2012 at 9:30 AM. The talk will be held at the New Hackensack Reformed Church in Wappingers Falls, N.Y. Dr. Wolter will provide current thoughts on these eye problems and for seniors and be available to answer questions. Statins May Prevent Cataracts

New York Cataract Surgeon Satish Modi, M.D. of Seeta Eye Centers in Poughkeepsie, N.Y. reviewed recent research indicating that taking statins may help prevent cataracts. ”We are always studying the current scientific literature and publications regarding cataracts, cataract surgery and lens implants so that we can use that knowledge to help our patients preserve their eye health and vision,” commented Dr. Modi. “According to researchers publishing their work in the February 2012 edition of the American Journal of Ophthalmology, the long term use of statins may have a protective effect against cataracts, especially for younger patients aged 50 to 64 years. This might be due to the antioxidant properties,” further explained Dr. Modi. Patients taking Lipitor (Atorvastin), Vytorin (Ezetimibe-Simvastatin), Mevacor (Lovastatin), Pravchol (Pravastatin) or Zocor (Simvastatin) for at least 5 years had a protective effect against cataract surgery among patients aged 50 to 64 years but not among patients older than 64 years. In both age groups, shorter-term use of less than 5 years was linked to an increased risk of cataracts. “The jury is still out on how or the significance of statins preventing cataracts-time will tell and we will continue to review the research,” concluded Dr. Modi.

Cataract Surgery Complications

New York Cataract Surgeon Satish Modi, M.D. discussed cataract surgery complications.

“For routine cataract surgery in healthy patients the complication rate is quite minimal-in fact well under 5%. Of course this can depend on the experience level of the cataract surgeon as well as whether you have diabetic eye disease, glaucoma or other types of health or eye problems,” explained Dr. Modi of Seeta Eye Centers in Poughkeepsie, N.Y. “Further, we are now using the bladeless laser cataract surgery technique, in addition to doing everything we would normally do, to reduce the risk of complications even more. Considering the significant vision and lifestyle improvements that cataract surgery and lens implants deliver for patients, the risk of complications is really quite low,” emphasized Dr. Modi. 

Even though cataract surgery complications are typically pretty minimal, it is important that patients thinking about any type of eye surgery have some idea of the types of complications that could possibly be encountered. 

Within the first 24-48 hours after a cataract operation, it is possible, but very unusual to experience some bleeding inside the eye, a little bleeding on the surface of the eye or an inner eye infection which is also quite rare. Sometimes, if your cataract surgeon needed to anesthetize the eye using an injection you may have a little “black and blue” eye or an eyelid bruise. This usually clears up pretty quickly. Sometimes the “capsule” around the crystalline lens that is removed as part of the operation might be broken during the surgery and it might be necessary to do a “vitrectomy” as part of the surgery to get a good result. This too is pretty unusual. In very nearsighted patients, although again infrequently, a retinal detachment might occur. Occasionally in patients who have had “no stitch” cataract surgery the actual incision may leak. If this happens it is sometimes necessary for your cataract surgeon to put a stitch in place to tightly close the incision. “With bladeless laser cataract surgery we are able to make absolutely perfect incisions with the laser each and every time we operate-making the possibility of this type of wound leak even more remote,” noted Dr. Modi. 


There are a few types of complications that can occur over a longer period of time after cataract surgery. These include that very rarely the intraocular lens implant (IOL) can move off center or the center of the retina can swell. By far the most common side effect is a clouding of the lens capsule that has been left in place to support the lens implant. "This is an eventual side effect of surgery that happens after virtually every cataract procedure. If one waits long enough, it happens after every cataract surgery, its just a question of when," remarked Dr. Modi. This side effect is pretty easily managed right in the office with a quick, painless procedure called a YAG Laser Capsulotomy that restores the vision almost immediately after the procedure.

Although it might sound like there are lots of possible complications of cataract surgery, rest assured they are quite unusual. This most common one of capsule opacity is easily treated. Eye Health, Vision & Food

NY Ophthalmologist Andreas Wolter, M.D., reminded us about the importance of how food choices and eating properly can affect eye health and vision. “Almost everyone is aware that diet is an important consideration in overall health-especially as related to cardiovascular problems, blood pressure problems and certainly in keeping our arteries healthy,” said Dr. Wolter of Seeta Eye Centers in Poughkeepsie, New York. “I like to discuss with patients-especially seniors-that maintaining eye health and vision depend on the very tiny blood vessels in the eye being able to supply oxygen and nutrients to the delicate tissues-especially in the retina. Keeping those blood vessels healthy is an important part of preserving vision as we age. As part of an overall healthy diet, several key nutrients appear to be particularly important to preserving sight as we get older", Dr. Wolter further explained. 

Vitamin C, Vitamin E & Zinc 

Over 10 years ago the Age-Related Eye Disease Study (AREDS) reported that people given vitamin E, vitamin C, beta carotene, and zinc in supplements were less likely to develop advanced age-related macular degeneration, or AMD. The combination was most effective at slowing the progression from intermediate to advanced AMD, which is one of the leading causes of age-related blindness. Because of those findings, many people diagnosed with early signs of AMD today are routinely prescribed a pill that combines these nutrients. You may or may not benefit from depending on your overall health and eye condition.
Lutein and Zeaxanthin

Two other nutrients-lutein and zeaxanthin-are also linked to lower risk of macular degeneration and cataracts. A 2008 Tufts University study of 1,802 women 50 to 79 years old found that those who consumed the most lutein and zeaxanthin in their diets were 23% less likely to develop cataracts than those who consumed the least. Rich sources of these two compounds include kale, spinach, turnip greens, romaine lettuce, broccoli, and Brussels sprouts. 

Omega-3 Fatty Acid

The newest nutrient linked to better vision with age is Omega-3 fatty acid, which is found predominantly in fish oil. In a study of 2,520 people, researchers at Johns Hopkins University reported in 2010 that people who consumed fish high in Omega-3s fatty acids often were significantly less likely to have advanced age-related macular degeneration. Omega-3s may also protect against cataracts, according to 2010 findings by researchers at the Clinical University of Navarra in Spain. Researchers don’t understand exactly why omega-3s may protect against eye diseases. One guess is that these healthy oils may reduce inflammation and thereby protect against cell damage. 

Pills vs. Food

Although antioxidant supplements are routinely prescribed to people with early signs of macular degeneration, there’s little agreement on whether supplements will help otherwise healthy people preserve their vision. Most of the evidence to date is very mixed. In one recent study, for example, a multivitamin seemed to protect against some forms of cataracts but actually raise the risk of other forms. 

PATIENTS SHOULD NOT BEGIN TAKING SUPPLEMENTS WITHOUT FIRST DISCUSSING IT WITH THEIR EYE DOCTOR AND INTERNIST OR FAMILY PHYSICIAN. 

Eating a healthy diet with healthy food choices is a good plan to help you maintain your eye health and vision as you age. For certain, eating more vegetables-including cabbage, broccoli, pepper, corn, or spinach-improved the condition of the retina in people with age-related macular degeneration.

Cataracts are the leading cause of blindness worldwide. About 20.5 million Americans age 40 and older have cataracts and more than half of all Americans develop cataracts by age 80. Many patients do not actually know that a cataract is a clouding of the crystalline lens of the eye preventing light rays from passing through it easily. This results in a clouding and blurring of vision. Patients should understand that cataracts are not a growth or a film over the eye. For many patients cataracts start out slowly and have little effect on vision at first. But, as the cataract becomes denser, so does the impact on vision. The most common symptoms that bother patients with cataracts include blurring of vision, sensitivity to light and glare, double vision in one eye, poor night vision, fading or yellowing of colors and the need for frequent changes in glasses or contact lens prescriptions.
